Smoke and Sound Vendor Registration

Vendor will abide by and be subject to the following agreement as

a condition for acceptance as in the Smoke & Sound Festival Market.

I am at least 18 years of age.

I understand that all applicants must be approved by the Monett Main Street Committee before the event.  If not approved, any payment already paid will be returned promptly. The Monett Main Street Committee reserves the right to deny participation to applicants based on the needs of the event.

I understand that the Monett Main Street Committee will make the assignments of booth locations for each exhibitor at their sole discretion. 

Payment can be made upon application approval.  Communication will follow.

I understand that I will need to supply my own tent/canopy.

I understand that I am responsible for collection and reporting of all sales taxes.

I understand that insurance for loss or damage of my products, exhibits or personal property or for loss or injury caused by products sold or exhibited by vendors is my responsibility.

I agree to hold the Monett Main Street Committee and Repurposed Faire Festival harmless of any loss, injury or damage.

I understand that following my application’s approval, I will receive further details about my participation in the Smoke & Sound Festival Market.

If this application is approved, I give permission to use my name, business/organization name and contact information for publicity purposes.

I have read the contract and agree to abide by these rules.

Please note, all food vendors must have appropriate health and business license documents prior to the event. Please attach health inspection and business license documentation to this application.
